They're really two different settings built for two different levels of need, and getting that match right is what spares your parent a hard move later.

An assisted living facility is for an older adult who needs help with the rhythms of daily life, things like bathing, dressing, medications, meals, and getting around, but who doesn't need ongoing skilled nursing.2 In Louisiana, these facilities are licensed as Adult Residential Care Providers (ARCPs) by the Louisiana Department of Health (LDH), Health Standards Section, under the minimum licensing standards in the Louisiana Administrative Code. The state uses four ARCP levels, which differ in the services they may offer and in their physical setting: Level 1 serves two to eight residents in a setting designed like a single-family home; Level 2 serves nine to 16 in a congregate setting without independent apartments equipped with kitchenettes; Level 3 serves 17 or more in independent apartments with kitchenettes; and Level 4 is the same as Level 3, but its care also includes intermittent nursing services.1

A nursing home, by contrast, is for someone who needs ongoing nursing care, with skilled nursing services and therapies as needed and 24-hour supervision; its services focus more on medical care than most assisted living facilities.2 That is a level of care a Louisiana assisted living facility isn't licensed to provide: no ARCP may serve a resident whose condition is so unstable that it requires continuous monitoring by licensed professional staff.1 Louisiana nursing facilities are licensed by that same LDH Health Standards Section, which also takes complaints against health care facilities, and families can compare federal results, including the CMS five-star Overall, Health Inspection, Staffing, and Quality Measure ratings, on Medicare's Care Compare tool, which LDH itself points families to.5 The threshold that moves someone from one setting to the other is that nursing-facility level of care: when a person's needs reach the point of requiring routine skilled nursing, an assisted living facility is usually no longer the right place, and a nursing home is.3,1

Who Each Setting Is Right For

If your parent is managing most of their day on their own but needs a steadier hand, help remembering medications, a little support with bathing or dressing, meals they don't have to cook, and people around so they're not isolated, an assisted living facility is usually the right fit. The setting is designed for exactly that: daily-living support without the medical intensity of a nursing home. Only Level 4 facilities may provide intermittent nursing services, and a Level 4 resident whose health needs increase may be able to stay and receive them, within the limits the state's rules set.1

A nursing home becomes the right setting when the care need crosses into skilled nursing: ongoing medical treatment, complex conditions that need nursing attention day and night, rehabilitation after a serious hospital stay, or the level of decline where round-the-clock supervision is the only safe option.2 Louisiana Medicaid funds this care for people who meet that nursing-facility level of care, which works as both a clinical bar and the gateway to coverage.3

One thing worth saying plainly: needs change. A parent who moves into assisted living today may, in a few years, reach the point where a nursing home is the safer place. That isn't a failure of the first choice. It's the normal arc of aging, and planning for it now, knowing the threshold and knowing how each setting is paid for, makes the eventual move far less wrenching than being caught off guard.

Assisted Living vs. Nursing Home in Louisiana: What Each Costs and Who Pays

This is where the decision gets real, so let's be plain about the numbers and where they come from.

In CareScout's 2025 Cost of Care Survey, fielded July through November 2025 and released March 2, 2026, the median cost of assisted living in Louisiana was about $61,950 a year, roughly $5,163 a month, based on 12 months of care in a private one-bedroom unit and well below the national median. At 365 days of care, a semi-private nursing home room ran about $91,250 a year, and a private room about $96,908 a year.4 These are industry-survey medians, not government rates, so treat them as a starting point for a budget rather than a quote. CareScout collects rates market by market and defines ten Louisiana regions, among them New Orleans and Metairie, Baton Rouge, Shreveport and Bossier City, and Lafayette, and it cautions that real costs vary with individual care needs, provider availability, and local market conditions.4

Louisiana's medians run below the national figures in all five categories CareScout's Louisiana annual table reports. Its nursing-home costs sit well below the 2025 national medians of about $114,975 for a semi-private room and about $129,575 for a private one, and its assisted living also runs below the national figure of about $74,400. Louisiana ranks 45th for assisted living, 46th for a semi-private room, and 45th for a private room on CareScout's cost ranking, where 1 is the highest.4 A nursing home still costs noticeably more per year than assisted living, though. The cost gap isn't the whole story, because the two settings are paid for in completely different ways, and that often matters more than the sticker price.

For assisted living, plan to pay privately unless a waiver helps. Louisiana's licensing rules contemplate a resident enrolled in a Medicaid home and community-based waiver that includes the facility as a service, but don't assume a given facility takes one. Budget for that roughly $5,163 a month from your parent's own income and savings, or long-term care insurance if they have it.4,1 Louisiana Medicaid's main home and community-based option is the Community Choices Waiver (CCW), the state's primary waiver for people 65 and older and adults with physical disabilities ages 21 to 64 who meet a nursing-facility level of care and qualify for long-term care Medicaid, letting them receive care at home instead of in an institution. The CCW pays for services such as personal assistance and home-delivered meals, and entry is gated by an offer: apart from the priority groups LDH names, offers go out first-come, first-served by the date someone asked for services, so a slot is not guaranteed the way nursing-home coverage is.6 If you've been picturing Medicaid covering the full cost of assisted living, check that assumption early.

One more payer to clear up, because families count on it and it doesn't apply here: Medicare pays for neither setting long-term. Medicare Part A covers a skilled nursing facility only on a short-term, post-acute basis, generally after a qualifying three-day inpatient hospital stay, up to 100 days per benefit period (days 1 through 20 at $0 a day after the $1,736 Part A deductible, which is normally already paid if the qualifying hospital stay fell in the same benefit period; days 21 through 100 with a $217-a-day coinsurance in 2026, and nothing after day 100). It does not pay for long-term custodial care in either an assisted living facility or a nursing home.7 For an ongoing long-term stay, families rely on private funds, long-term care insurance, VA benefits, or Louisiana Medicaid.7

A nursing home is covered by Louisiana Medicaid for those who qualify. Louisiana Medicaid (Healthy Louisiana) covers nursing-home care for people who meet a nursing-facility level of care and the financial rules. For a single applicant in 2026, the income limit is $2,982 a month, three times the 2026 Supplemental Security Income (SSI) federal benefit rate of $994, and the countable-resource limit is $2,000, while a spouse who stays at home is protected by a resource allowance computed against 2026 federal standards of $32,532 minimum and $162,660 maximum.3 A nursing-home resident's income is applied to the cost of care, and Louisiana sets the personal care needs allowance at $45 a month, effective July 1, 2025.3

A couple of things to plan around, because they can change whether and when someone qualifies. Louisiana applies a 60-month look-back at resources that were disposed of or placed in a trust before the application. And after the death of someone who received long-term care, home and community-based services, or related hospital and prescription drug services after age 55, the state pursues estate recovery, but federal law limits when it can happen: only after a surviving spouse has died, and only while there is no surviving child who is under 21, or blind or permanently and totally disabled. LDH places the case in deferred status while a spouse survives and reopens it when that spouse dies. Relief for a low-income heir isn't automatic, though. LDH's long-term care page says recovery is waived for a first-degree heir whose family income was at or below 300 percent of the federal poverty level in the year before the death, but the state's Medicaid plan defines undue hardship as compelling circumstances that would place an unreasonable burden on the surviving spouse or an heir and family income at or below that level, so be ready to document both. The heirs must prove the hardship, and the executor or authorized representative has 30 days from the mail date of LDH's estate recovery notice to file a completed hardship waiver application with supporting documents.3 If your parent's income or assets are anywhere near the line, it's worth understanding the rules before anyone applies. How to Decide

When you strip it down, the decision rests on those same two questions, in this order.

Two more practical notes. First, plan for the move between the two settings. Many families start in assisted living and shift to a nursing home as needs rise, so it helps to know in advance what your parent's resources would cover in each, and what Medicaid would and wouldn't pick up.
