The income standard is $994 per month for a household of one ($1,491 for a household of two) in 2026, and above it Hawaii imposes no hard income cap at all. It is a medically needy state, so an applicant over that standard can still qualify: income above the medically needy income limit of $469 per month for a household of one ($632 for a household of two) is spent down on care each month instead of disqualifying them. The spend-down covers income only: those $2,000 and $3,000 resource limits still apply on the medically needy pathway. VA Program of Comprehensive Assistance for Family Caregivers (PCAFC)

The PCAFC pays a monthly stipend to the Primary Family Caregiver of an eligible veteran. It is federal tax-free and allows paid spouses. To qualify, the veteran needs a service-connected disability rating of 70 percent or higher, a need for in-person personal care for at least six continuous months, and enrollment in VA health care. The stipend starts from the federal GS-4, Step 1 annual rate for the veteran's locality, divided by 12. That monthly base is then multiplied by one of four factors set at 38 CFR 71.40(c)(4)(i), and which one applies depends on whether the veteran is in the current program or is a legacy participant or legacy applicant.

  • Current program (38 CFR 71.20(a)): the factor is 0.625, or 1.00 if VA determines the veteran is unable to self-sustain in the community.
  • Legacy participant or legacy applicant (38 CFR 71.20(b) or (c)): the factor is set instead by the sum of the veteran's 2019 clinical ratings, with no self-sustain determination required. A sum of 21 or higher pays 1.00, a sum of 13 to 20 pays 0.625, and a sum of 1 to 12 pays 0.25. This legacy schedule lapses October 1, 2028.

A veteran who meets both sets of criteria is paid whichever of the two amounts is higher. A legacy participant's stipend also cannot fall below what the caregiver was eligible to receive the day before October 1, 2020, for as long as the veteran remains at the address VA has on record.

VA Aid and Attendance Pension

Wartime veterans and surviving spouses who meet the functional criteria and have countable assets and income under the net-worth limit ($163,699 in 2026) may receive the Aid and Attendance pension. VA sets these rates as an annual maximum and pays only the difference between a claimant's countable income and that maximum, so someone with income receives less: a single veteran with Aid and Attendance can receive up to $29,093 a year (roughly $2,424 a month); a veteran with one dependent up to $34,488 a year (roughly $2,874 a month). The pension goes to the veteran, who typically pays a family caregiver from it.
