How People Pay for Senior Care in Virginia
There are five main ways to pay. Each one covers something different. Most families use a mix.
| Payer | What It Does | What It Doesn't Do |
|---|---|---|
| Out of pocket | Pays for any care, anywhere, right away | Drains savings fast at Virginia prices |
| Medicare | Up to 100 days of skilled care after a hospital stay | No long-term assisted living or custodial nursing home care |
| Medicaid (Cardinal Care) | The main payer for long-term nursing home and home care | Income and asset limits apply |
| VA Aid and Attendance | Extra monthly cash for wartime veterans and surviving spouses | Only for those who qualify by service and need |
| Long-term care insurance | Pays toward home care, assisted living, and nursing homes | Only if a policy was bought years earlier |
What Senior Care Costs in Virginia
Know what you're paying for before you work out how to pay. The figures below are Virginia statewide medians from the CareScout (Genworth) 2025 Cost of Care Survey, released March 2, 2026. These are industry-survey medians, not government figures. Virginia costs now run above the national medians for most care settings, after sharp year-over-year jumps in nursing-home rates.Genworth Financial. (2025). CareScout Releases 2025 Cost of Care Survey Results. investor.genworth.com. Retrieved Jun 25, 2026, from https://investor.genworth.com/news-events/press-releases/detail/1054/carescout-releases-2025-cost-of-care-survey-results
| Care Type | Median Cost | What It Is |
|---|---|---|
| In-home care (non-medical caregiver) | $80,080/year ($6,673/month) | Non-medical help with daily activities, meals, and errands |
| Adult day care | $20,995/year ($1,750/month) | Daytime supervision and activities |
| Assisted living | $83,328/year ($6,944/month) | Room, board, and personal care |
| Nursing home (semi-private) | $123,005/year ($10,250/month) | Skilled, around-the-clock care |
| Nursing home (private room) | $140,160/year ($11,680/month) | Private room, skilled care |
Northern Virginia runs higher than these medians. Rural parts of the state run lower. A two-year nursing home stay at the statewide median already passes $246,000.Genworth Financial. (2025). Retrieved Jun 25, 2026, from https://investor.genworth.com/news-events/press-releases/detail/1054/carescout-releases-2025-cost-of-care-survey-results That is why families combine the funding sources below. Paying Out of Pocket
Most families start here. They draw on Social Security, pensions, retirement savings, and home equity. For a while it may be the only option, especially before Medicaid comes through.
A few private-pay tools Virginia families use:
- Home equity. Sell the home, rent it out, or borrow against it. A reverse mortgage (for owners 62 and older) turns equity into cash. But the home is usually exempt for Medicaid, so weigh any move against a future Medicaid plan first.U.S. Social Security Administration. (2016). SSA - POMS: SI 01715.020 - List of State Medicaid Programs for the Aged, Blind and Disabled - 08/02/2016. secure.ssa.gov. Retrieved Jun 24, 2026, from https://secure.ssa.gov/poms.nsf/lnx/0501715020
- Life insurance. Some policies pay an accelerated death benefit for a terminally ill policyholder, or can be sold in a life settlement.
- Retirement and investment accounts. The most flexible source. They are also countable assets for Medicaid, so spending them down has consequences later.
The hard truth: paying out of pocket at Virginia prices drains savings fast. Treat private pay as a bridge. Our guides to paying for in-home care and paying for assisted living cover the options in more depth.
What Medicare Does and Doesn't Cover
This is where families get caught off guard. Medicare does not pay for long-term care. It will not cover the ongoing help with bathing, dressing, eating, and supervision that most seniors eventually need.Centers for Medicare & Medicaid Services. (n.d.). SNF Care Coverage. medicare.gov. Retrieved Jun 23, 2026, from https://www.medicare.gov/coverage/skilled-nursing-facility-care
What Medicare does cover is limited and medical:
- Skilled nursing facility care for up to 100 days after a qualifying three-day hospital stay. Days 1 through 20 are covered in full. Days 21 through 100 require a daily coinsurance. After day 100, nothing.Centers for Medicare & Medicaid Services. (n.d.). SNF Care Coverage. medicare.gov. Retrieved Jun 23, 2026, from https://www.medicare.gov/coverage/skilled-nursing-facility-care
- Home health care when a doctor orders skilled nursing or therapy and the person is homebound.
- Hospice care for someone who is terminally ill.
What Medicare never covers: long-term nursing home stays, assisted living, adult day care, and non-medical home care. For ongoing care, Virginians rely on Medicaid, VA benefits, insurance, or private pay.
Virginia Medicaid: The Main Way to Pay for Long-Term Care
Virginia Medicaid, branded Cardinal Care, is the dominant payer for long-term senior care in the state. The Virginia Department of Medical Assistance Services runs it, and your local Department of Social Services processes applications.U.S. Social Security Administration. (2016). SSA - POMS: SI 01715.020 - List of State Medicaid Programs for the Aged, Blind and Disabled - 08/02/2016. secure.ssa.gov. Retrieved Jun 24, 2026, from https://secure.ssa.gov/poms.nsf/lnx/0501715020
Cardinal Care covers nursing home care and home care for people who qualify financially and clinically.
How Medicaid Pays for Care at Home
Most families would rather keep a parent at home than move them into a facility. Virginia pays for that through home and community-based services (HCBS) delivered under the Commonwealth Coordinated Care Plus (CCC Plus) waiver, run through Cardinal Care managed care: a member enrolls in a managed-care health plan that coordinates their long-term services and supports. The waiver can pay for personal care, adult day services, respite, and other supports that let someone stay in their own home instead of a nursing facility.U.S. Social Security Administration. (2016). SSA - POMS: SI 01715.020 - List of State Medicaid Programs for the Aged, Blind and Disabled - 08/02/2016. secure.ssa.gov. Retrieved Jun 24, 2026, from https://secure.ssa.gov/poms.nsf/lnx/0501715020
The financial rules are the same as for nursing-facility Medicaid: the $2,982 monthly income standard (300% of SSI) and the $2,000 asset limit both apply to waiver applicants. But home-care slots are not always available on demand. Waiver capacity can be limited, so ask your local Department of Social Services about current availability and any wait when you apply, and line up private pay or family care to bridge any gap.U.S. Social Security Administration. (2016). SSA - POMS: SI 01715.020 - List of State Medicaid Programs for the Aged, Blind and Disabled - 08/02/2016. secure.ssa.gov. Retrieved Jun 24, 2026, from https://secure.ssa.gov/poms.nsf/lnx/0501715020
Who Qualifies in 2026
Virginia is a medically needy state, not a strict income-cap state. It does not require a Miller Trust.U.S. Social Security Administration. (2016). SSA - POMS: SI 01715.020 - List of State Medicaid Programs for the Aged, Blind and Disabled - 08/02/2016. secure.ssa.gov. Retrieved Jun 24, 2026, from https://secure.ssa.gov/poms.nsf/lnx/0501715020
- Assets: up to $2,000 in countable assets for a single applicant in 2026, and $4,000 for a couple with both applying. The home, one vehicle, and personal belongings are typically exempt.U.S. Social Security Administration. (2016). SSA - POMS: SI 01715.020 - List of State Medicaid Programs for the Aged, Blind and Disabled - 08/02/2016. secure.ssa.gov. Retrieved Jun 24, 2026, from https://secure.ssa.gov/poms.nsf/lnx/0501715020
- Income: the standard for nursing-facility and waiver coverage is $2,982 a month (300% of the 2026 SSI benefit rate). An applicant over it can still qualify by spending down the excess on medical and care costs.
- Spousal protection: the at-home spouse can keep half the couple's countable assets up to $162,660 (the Community Spouse Resource Allowance) so they are not left with nothing.
Virginia also applies a 60-month look-back. Assets given away or sold for less than fair value in the five years before applying can trigger a penalty period.U.S. Government Publishing Office. (2023). 42 USC 1396p - Liens, adjustments and recoveries, and transfers of assets (govinfo, U.S. Code). govinfo.gov. VA Aid and Attendance for Veterans
If your loved one is a wartime veteran or the surviving spouse of one, VA Aid and Attendance can be a real funding source. It is an extra monthly amount added to the VA pension for veterans who need help with daily activities or are housebound. The money can pay for home care, assisted living, or a nursing home.U.S. Department of Veterans Affairs. (n.d.). Current Pension Rates For Veterans. va.gov. Retrieved Jun 24, 2026, from https://www.va.gov/pension/veterans-pension-rates/
For the rate year that began December 1, 2025, the maximum monthly amounts are:
- Single veteran: up to $2,424 a month.U.S. Department of Veterans Affairs. (n.d.). Current Pension Rates For Veterans. va.gov. Retrieved Jun 24, 2026, from https://www.va.gov/pension/veterans-pension-rates/
- Veteran with a spouse or dependent: up to $2,874 a month.
- Surviving spouse: up to $1,558 a month.
Aid and Attendance is need-based. The actual payment is the maximum rate minus countable income, after subtracting unreimbursed medical costs. The 2026 net worth limit is $163,699, and there is a three-year look-back on asset transfers.U.S. Department of Veterans Affairs. (n.d.). Current Pension Rates For Veterans. va.gov. Retrieved Jun 24, 2026, from https://www.va.gov/pension/veterans-pension-rates/ It can work alongside Medicaid in some cases, so check it early. Virginia has one of the largest veteran populations in the country, so this is worth checking for many families here.
Long-Term Care Insurance
Did your family member buy a long-term care policy years ago? Dig it out now. Read the benefit triggers, the daily maximum, and the waiting period before you need them. These policies typically pay toward home care, assisted living, and nursing home care up to a set daily or monthly amount.
Some older policies tie to the federal and state Long-Term Care Partnership Program, which links approved policies to extra Medicaid asset protection. Dollars the policy pays out are dollars you can keep and still qualify for Medicaid later, on top of Virginia's standard $2,000 asset limit.U.S. Social Security Administration. (2016). SSA - POMS: SI 01715.020 - List of State Medicaid Programs for the Aged, Blind and Disabled - 08/02/2016. secure.ssa.gov. Retrieved Jun 24, 2026, from https://secure.ssa.gov/poms.nsf/lnx/0501715020 New policies are expensive and hard to qualify for after 65, so this is mainly a tool for people who bought in earlier. If a policy exists, treat it as central, and time a Medicaid application around when its benefits run out. Our guide to long-term care insurance explains how these policies work.
Other Ways to Pay for Senior Care in Virginia
A few smaller levers can stretch a budget. None replaces the main sources above. Get advice before acting on the ones with long-term consequences.
- Auxiliary Grant. Virginia pays a monthly supplement to some low-income residents of approved assisted living facilities and adult foster care, on top of their Social Security, to help with room and board.
- Annuities and trusts. Medicaid-compliant annuities and certain irrevocable trusts can reposition assets. The 60-month look-back and Virginia's specific rules make these easy to get wrong.U.S. Government Publishing Office. (2023). 42 USC 1396p - Liens, adjustments and recoveries, and transfers of assets (govinfo, U.S. Code). govinfo.gov. Retrieved Jun 23, 2026, from https://www.govinfo.gov/content/pkg/USCODE-2023-title42/html/USCODE-2023-title42-chap7-subchapXIX-sec1396p.htm Talk to an elder-law attorney first.
- A written funding plan. Combining sources in the right order is the whole game. Our guide to building a senior care funding plan walks through how to sequence them.
