Original Medicare: Parts A and B
Original Medicare is run directly by the federal government and has two parts. The mechanics and costs are identical in South Dakota and every other state.
Part A (Hospital Insurance)
Part A covers inpatient hospital stays, limited skilled nursing facility care, hospice, and some home health care.Centers for Medicare & Medicaid Services. |---|---|
| Monthly premium | $0 for most people (40+ quarters of work history) |
| Hospital deductible | $1,736 per benefit period |
| Hospital coinsurance, days 61-90 | $434 per day |
| Lifetime reserve days | $868 per day |
| SNF coinsurance, days 21-100 | $217 per day |

Get readmitted after that and the deductible applies again.
Part B (Medical Insurance)
Part B covers doctor visits, outpatient care, preventive services, durable medical equipment, and mental health care. Medicare Advantage in South Dakota (Part C)
Medicare Advantage plans are an alternative to Original Medicare, sold by private insurers. They cover everything Parts A and B do, except hospice, which Original Medicare keeps covering. South Dakota residents in the Sioux Falls and Rapid City areas generally see the widest plan selection. Rural counties across the state tend to have fewer choices, and some areas may have very limited Medicare Advantage availability. Plan options, networks, and prices change every year and differ by county, so compare what's available at your address before you decide.
How These Plans Work
- You keep paying your Part B premium ($202.90) on top of any plan premium. The CMS estimate for the average Medicare Advantage premium in 2026 is about $14 a month, and many plans charge $0 extra. Some plans go further and help pay all or part of your Part B premium.Centers for Medicare & Medicaid Services. (2026). 2026 Medicare Parts A & B Premiums and Deductibles. cms.gov. - Plans run on networks (HMO or PPO). Confirm your doctors and hospitals are in-network before you enroll. Emergency care, out-of-area urgent care, and temporary out-of-area dialysis are covered whether you get them in the plan's network or outside it.Centers for Medicare & Medicaid Services. (n.d.). Retrieved Aug 8, 2026, from https://www.medicare.gov/basics/get-started-with-medicare/get-more-coverage/your-coverage-options/compare-original-medicare-medicare-advantage
- Plans usually require prior authorization for certain services, which Original Medicare generally does not.Centers for Medicare & Medicaid Services. (n.d.). Retrieved Aug 8, 2026, from https://www.medicare.gov/basics/get-started-with-medicare/get-more-coverage/your-coverage-options/compare-original-medicare-medicare-advantage
- Every plan caps your annual in-network out-of-pocket spending (federally limited to $9,250 in 2026; many plans set it lower). Original Medicare has no such cap on its own. CMS's sentence adds the condition: no yearly limit on what you pay out of pocket unless you have supplemental coverage, like Medigap, Medicaid, employer, retiree, or union coverage. What each covers varies by policy.Centers for Medicare & Medicaid Services. (n.d.). Compare Original Medicare & Medicare Advantage - Medicare.gov. medicare.gov. Medicare Part D: Prescription Drugs
Part D covers outpatient prescription drugs. You can get it as a standalone plan paired with Original Medicare, or built into a Medicare Advantage plan.

Medigap in South Dakota
Medigap, also called Medicare Supplement Insurance, is extra insurance you buy from a private health insurance company to help pay your share of out-of-pocket costs in Original Medicare. You can't use a Medigap policy to pay a Medicare Advantage plan's copayments, coinsurance, deductibles, or premiums.Centers for Medicare & Medicaid Services. (n.d.). 12026 understanding medicare advantage plans. medicare.gov. Retrieved Sep 26, 2026, from https://www.medicare.gov/publications/12026-understanding-medicare-advantage-plans.pdf
Medigap plans are standardized by letter. Medicare's 2026 guide Choosing a Medigap Policy charts them as Plans A, B, C, D, F, G, K, L, M, and N, and every insurance company that sells Medigap policies must offer at least Plan A.Centers for Medicare & Medicaid Services. Your strongest opening is the six-month Medigap Open Enrollment Period that begins with the first month you are 65 or older and enrolled in Part B. South Dakota law (SDCL 58-17A-17) protects an application submitted before or during that window: no insurer may deny, condition, or price a policy because of your health status, claims experience, receipt of health care, or medical condition, and each policy an insurer currently offers must be made available to you.Centers for Medicare & Medicaid Services. South Dakota's Medicare Supplement rules also cover people under 65 who have Medicare because of a disability. Under ARSD 20:06:13:84, any individual, regardless of age, who becomes eligible for Medicare by reason of age or disability is entitled to open enrollment into any Medigap policy if they apply within six months of enrolling in Part B; the rule applies only to people who became eligible for Medicare on or after July 1, 1999.Centers for Medicare & Medicaid Services. After certain coverage losses, South Dakota's guaranteed issue rules (ARSD 20:06:13:79 through 20:06:13:80.01) apply. They include situations such as employer coverage ending or a Medicare Advantage plan no longer serving your area, and for an eligible person an insurer may not deny certain Medigap policies, price them based on health status, or impose a pre-existing condition exclusion.Centers for Medicare & Medicaid Services. Medigap or Medicare Advantage?
You can't run both at once: you can't use a Medigap policy to pay your Medicare Advantage copayments, coinsurance, deductibles, or premiums, and while you're in an Advantage plan it's illegal for anyone to sell you a Medigap policy unless you're switching back to Original Medicare.Centers for Medicare & Medicaid Services. Retrieved Sep 26, 2026, from https://www.medicare.gov/publications/12026-understanding-medicare-advantage-plans.pdf The choice isn't always permanent. If you join a Medicare Advantage plan for the first time and aren't happy with it, federal law gives you a trial right to buy a Medigap policy and a separate Medicare drug plan if you return to Original Medicare within 12 months of joining.Centers for Medicare & Medicaid Services. (n.d.). 12026 understanding medicare advantage plans. medicare.gov. Retrieved Sep 26, 2026, from https://www.medicare.gov/publications/12026-understanding-medicare-advantage-plans.pdf South Dakota's guaranteed issue rules also cover someone who dropped a Medigap policy to enroll for the first time in a Medicare Advantage plan and ends that enrollment within the first 12 months.Centers for Medicare & Medicaid Services. Help Paying for Medicare in South Dakota
If you're on a fixed income, two programs can cut your Medicare costs sharply.
Medicare Savings Programs
South Dakota administers its Medicare Savings Programs through SD DSS Medicaid. The income limits below are the federal 2026 standards from the POMS row covering all states except Alaska and Hawaii, and DSS uses its own QI-1 label for the highest tier. DSS publishes that top threshold ($1,816 for an individual, $2,455 for a couple) and the resource limits itself; it does not publish a QMB or SLMB income figure of its own, so those two tiers are the federal standards rather than numbers the state states.U.S. Social Security Administration. (2026). SSA - POMS: HI 00815.023 - Medicare Savings Programs Income and Resource Limits - 02/26/2026. secure.ssa.gov. Retrieved Jun 24, 2026, from https://secure.ssa.gov/poms.nsf/lnx/0600815023
| Program | Federal name | Individual | Couple | What it pays |
|---|---|---|---|---|
| QMB | Qualified Medicare Beneficiary | Up to $1,350 | Up to $1,824 | Part A and B premiums, deductibles, coinsurance |
| SLMB | Specified Low-Income Medicare Beneficiary | Up to $1,616 | Up to $2,184 | Part B premium |
| QI-1 | Qualifying Individual-1 | Up to $1,816 | Up to $2,455 | Part B premium |
QMB is the most generous, covering your Part B premium plus your deductibles and coinsurance, and federal law bars providers from billing a QMB enrollee for that cost-sharing. QI has to be applied for again every year, because being selected one year does not carry you into the next.U.S. Social Security Administration. (2026). SSA - POMS: HI 00815.023 - Medicare Savings Programs Income and Resource Limits - 02/26/2026. secure.ssa.gov. That limit is not a cap on everything you own. DSS counts checking and savings accounts and certificates of deposit, and its brochure states that in figuring your savings you do not include your home, one car per couple used for transportation, burial plots, up to $1,500 for burial expenses, or a limited amount in an irrevocable burial trust.U.S. Social Security Administration. Retrieved Jun 24, 2026, from https://secure.ssa.gov/poms.nsf/lnx/0600815023 Each income figure above is that tier's Federal Poverty Level percentage plus the $20 monthly SSI general income exclusion, not the bare poverty-level amount, and DSS says the income and savings limits may change slightly each year.U.S. Social Security Administration. (2026). SSA - POMS: HI 00815.023 - Medicare Savings Programs Income and Resource Limits - 02/26/2026. secure.ssa.gov. Retrieved Jun 24, 2026, from https://secure.ssa.gov/poms.nsf/lnx/0600815023 None of these numbers is an absolute cutoff: states can effectively raise both the income and the resource limits by disregarding certain income and resources, so someone somewhat over should still apply. You apply through SD DSS online or at a local DSS office on 877.999.5612, and SHIINE volunteers fill out the Medicare Savings Program application with you free of charge.U.S. Social Security Administration. Retrieved Sep 2, 2026, from https://www.medicare.gov/health-drug-plans/open-enrollment If you're still working past 65 with group health coverage based on that current employment, you can delay Part B without penalty and use an eight-month Special Enrollment Period to sign up, counted from the month that job or coverage ends, whichever comes first; miss that window and you wait for the General Enrollment Period (January 1 to March 31) and may owe a late penalty. COBRA and retiree coverage do not count as coverage based on current employment, and if you're self-employed or your plan isn't available to everyone at the company, ask your insurer whether it qualifies before you delay; if it doesn't, sign up at 65. This Special Enrollment Period does not apply if you're eligible for Medicare based on End-Stage Renal Disease (ESRD).Centers for Medicare & Medicaid Services. (n.d.).
