In New Mexico, what most families call "assisted living" is licensed as an Assisted Living Facility for Adults, overseen by the New Mexico Health Care Authority through its Health Facility Licensing and Certification Bureau, under New Mexico Administrative Code 8.370.14 and Section 24-1-2 NMSA 1978. That rule took effect July 1, 2024, replaced an earlier, now-repealed rule, and the licensing job moved to the Health Care Authority, which now runs both the state's Medicaid program and its health-facility licensing. A facility has to hold this license to operate.1

A nursing home, by contrast, is for someone who needs skilled care by licensed nurses around the clock, the kind of medical support an assisted living facility isn't built or licensed to provide. New Mexico nursing facilities are licensed and inspected by that same Health Care Authority, through its Division of Health Improvement, and a facility that takes part in Medicare or Medicaid is also federally certified, with inspection results and a one-to-five-star rating published on Medicare's Care Compare tool.2 The threshold that moves someone from one setting to the other is that nursing-facility level of care: when a person's needs reach the point of requiring routine skilled nursing, an assisted living facility is usually no longer the right place, and a nursing home is.4

Who Each Setting Is Right For

If your parent is managing most of their day on their own but needs a steadier hand, help remembering medications, a little support with bathing or dressing, meals they don't have to cook, and people around so they're not isolated, an assisted living facility is usually the right fit. The setting is designed for exactly that: daily-living support without the medical intensity of a nursing home. In New Mexico, these homes are licensed as Assisted Living Facilities for Adults under the rule that took effect in 2024, and a current license is the floor to look for before you fall for any one place.1

A nursing home becomes the right setting when the care need crosses into skilled nursing: ongoing medical treatment, complex conditions that need licensed-nurse attention day and night, recovery from a serious hospital stay, or the level of decline where round-the-clock care is the only safe option.2 New Mexico Medicaid funds this care for people who meet that nursing-facility level of care, which works as both a clinical bar and the gateway to coverage.4

Assisted Living vs. Nursing Home Cost in New Mexico, and Who Pays

This is where the decision gets real, so let's be plain about the numbers and where they come from.

The most recent industry benchmark is the CareScout (formerly Genworth) 2025 Cost of Care Survey, released in March 2026. Nationally, it puts the median cost of assisted living at about $6,200 a month ($74,400 a year). A semi-private nursing home room runs about $114,975 a year ($315 a day), and a private room about $129,575 a year ($355 a day) (national medians).3 CareScout did not publish a separate New Mexico figure for 2025, but the state's facility costs have historically tracked at or slightly above these national medians, and in the prior 2024 survey New Mexico assisted living ran about $6,163 a month, modestly above the national figure.3 These are industry-survey medians, not government rates, so treat them as a budgeting starting point rather than a quote; costs vary across the state and rise as care needs grow.

Either way, a nursing home costs noticeably more than assisted living, roughly 1.5 times as much at the national median.3 The cost gap isn't the whole story, though, because the two settings are paid for in completely different ways, and that often matters more than the sticker price.

Assisted living is largely private-pay. New Mexico Medicaid does not pay an assisted living resident's room and board. That monthly assisted-living cost generally comes out of your parent's own income and savings, or long-term care insurance if they have it. There is one wrinkle worth knowing: New Mexico's Community Benefit, delivered under managed care now branded Turquoise Care, can cover assisted-living services such as personal care and supervision for residents who qualify, even though it won't pay the rent and meals.1 Unlike nursing-home care, this assisted-living help is not an automatic entitlement; it is delivered through a managed-care benefit with its own enrollment, so it can be limited and is never a substitute for the room-and-board cost. A nursing home is covered by New Mexico Medicaid for those who qualify. Nursing-facility care is a mandatory Medicaid benefit in every state, so once someone qualifies it is an entitlement, not a waitlisted program. New Mexico Medicaid, delivered through managed care now branded Turquoise Care (formerly Centennial Care) and administered by the Health Care Authority, covers nursing-home care for people who meet a nursing-facility level of care and the financial rules.4 For a single applicant in 2026, the monthly income limit is 300% of the Supplemental Security Income (SSI) federal benefit rate, about $2,982 a month (a qualifying income trust is required for someone over that), and the countable-asset limit is generally $2,000, with a larger resource allowance protected for a spouse who stays in the community (up to $162,660 in 2026).4 A nursing-home resident on New Mexico Medicaid pays most of their monthly income toward the cost of care and keeps a modest monthly personal needs allowance for personal expenses.4

A couple of things to plan around, because they can change whether and when someone qualifies. New Mexico enforces a 60-month look-back on assets given away or transferred for less than fair value, which can trigger a penalty period of ineligibility. And, as federal law requires, the state recovers from the estates of people who received long-term-care services at age 55 or older.4 If your parent's income or assets are anywhere near the line, it's worth understanding the rules before anyone applies. One thing Medicare does not do: pay for either setting long-term. Families often assume Medicare will cover a long nursing-home stay, but Medicare Part A pays only for short-term, post-acute skilled-nursing care, up to 100 days per benefit period after a qualifying hospital stay of at least three days, and it pays nothing toward long-term custodial care or assisted living.5 The vocabulary trips people up: the same building can be a short-term Medicare rehab "skilled nursing facility" and a long-term custodial nursing home, but Medicare only pays for the brief rehab stay. For the long stay, families rely on private pay, long-term care insurance, or, once someone qualifies, Medicaid.5
