Kentucky family caregivers can access funded respite through PDS waivers, free NFCSP grants through 15 Area Agencies on Aging, and adult day programs. Call 1-800-372-2991 to find what you qualify for.
Caregiving without planned breaks is unsustainable. Research consistently shows that family caregivers who skip regular relief have higher rates of depression, immune problems, and burnout than the general population. Practically speaking, caregivers who build in respite keep their loved ones at home longer than those who do not. A break isn't giving up. It is what makes long-term care possible.
This guide maps every funded respite option in Kentucky for 2026.
Why Respite Matters
Kentucky has hundreds of thousands of unpaid family caregivers, most of them providing daily or near-daily help with personal care, medication management, or mobility. Most do it without scheduled relief and without recognizing that their own health is eroding in the process.
Respite addresses this directly. In-home respite gives the care recipient supervision and companionship while the caregiver rests, runs errands, or sleeps. Adult day services provide structured daytime programming, meals, and peer engagement. Short-stay facility respite opens a longer window when the primary caregiver needs to recover from illness, travel, or simply recharge.
Kentucky has publicly funded programs for caregivers at every income level.
Funded Respite Options in Kentucky
1. Participant Directed Services (PDS) Waivers
What it is: Participant Directed Services (PDS) is Kentucky's self-directed option within the state's Medicaid 1915(c) waivers, administered by the Department for Aging and Independent Living (DAIL) under the Department for Medicaid Services (DMS). The participant acts as the employer: they hire, manage, and direct their own workers, and respite is a covered service within the PDS plan.
Which waivers offer PDS? PDS is available through four waivers: the Home and Community Based (HCB) waiver for aging and physical disabilities, the Supports for Community Living (SCL) and Michelle P. waivers for intellectual or developmental disabilities, and the Acquired Brain Injury (ABI) waivers. A PDS case manager or support broker helps develop the plan of care and budget.
Who handles payroll? A Financial Management Services (FMS) agency handles payroll processing, tax withholding, and worker background checks. The participant remains the employer of record and directs the work; the FMS agency processes the pay.
Who can be the paid respite worker? Adult children, siblings, other relatives, friends, and neighbors are all eligible to be hired as PDS workers. A spouse or other legally responsible individual (LRI) generally cannot be paid, with one narrow exception: an LRI may be paid when they provide extraordinary or specialized care beyond what a typical caregiver provides, subject to documentation requirements. This exception took effect July 1, 2024.
How to access: Contact the Kentucky CHFS DAIL or your local Area Development District office to begin a Medicaid waiver assessment.
Best for: Medicaid-eligible Kentuckians who want to self-direct respite care and hire a family member (other than a spouse or LRI in most cases) or trusted person.
2. NFCSP Grants Through Kentucky's 15 Area Agencies on Aging
What it is: The National Family Caregiver Support Program (NFCSP), funded by Title III-E of the Older Americans Act, flows through the Kentucky Cabinet for Health and Family Services, Division of Aging Services, to 15 regional Area Agencies on Aging. Each AAA provides in-home respite, adult day vouchers, caregiver training, counseling, and supplemental services.
Who qualifies: Family caregivers of adults age 60 and older; grandparent and relative caregivers of children under 18; caregivers of any age caring for someone with Alzheimer's disease or a related dementia. No income test for NFCSP respite services.
How to access: Call the CHFS helpline at 1-800-372-2991 or use the Eldercare Locator at 1-800-677-1116. Both connect you to your regional AAA.
Best for: Any Kentucky family caregiver, regardless of income. NFCSP is consistently underused statewide.
3. Adult Day Programs
What they are: Adult day and adult day health centers provide structured daytime programming, typically 4 to 8 hours a day, with meals, activities, social engagement, and varying levels of nursing support. For caregivers of people with dementia, consistent adult day attendance often reduces behavioral symptoms and helps make sustained home care more manageable.
Finding programs: The Kentucky Cabinet for Health and Family Services licenses adult day programs statewide. Your regional AAA maintains a local directory. Call 1-800-372-2991 for referrals in your area.
Who pays:
- NFCSP grants through your AAA can offset adult day costs.
- PDS waiver can cover adult day for enrolled members who include it in their plan.
- Long-term care insurance often covers adult day under the home care rider.
- Private pay rates vary by center.
Best for: Caregivers who need consistent weekday relief and care recipients who benefit from structured activity and social engagement.
4. VA Respite for Veterans
What it is: Veterans enrolled in VA health care can access respite through their local VA medical center, including in-home aide services, adult day health, and short-term placement in VA Community Living Centers or contracted facilities.
PCAFC: Primary Family Caregivers in the Program of Comprehensive Assistance for Family Caregivers receive respite care as part of the benefit package; confirm authorized services and any cost-sharing with the VA Caregiver Support Coordinator.
How to access: Call the VA Caregiver Support Line at 1-855-260-3274 or contact the Caregiver Support Coordinator at the Lexington VA Medical Center, the Louisville VA Medical Center, or your nearest VA facility.
Best for: Families of veterans enrolled in VA health care.
