Which Oklahoma Caregiver Programs Pay Family Members?

For a lot of families, this is the real question: can the person already doing the caregiving get paid for it? In Oklahoma, the answer is often yes, and it runs mostly through one program.

ADvantage Waiver CDPASS: The Paid Route

Oklahoma's ADvantage waiver is a 1915(c) Medicaid home and community-based services program that helps eligible SoonerCare members stay home rather than enter a nursing facility. Its Consumer Directed Personal Assistance Services and Supports (CDPASS) option puts the member in charge: the member becomes the employer and hires, trains, and schedules their own personal support assistant.2

Here is the part that matters if you are a husband or wife doing the caregiving yourself. Under CDPASS, a legally responsible spouse or a legal guardian may be authorized as a paid provider as a policy exception. When a spouse or guardian is hired, the case manager must visit the member at least monthly to confirm the arrangement remains appropriate. So the door many states close to spouses is open in Oklahoma, with monitoring attached.1

Do You Qualify Financially? SoonerCare Limits

Because CDPASS runs through SoonerCare, your loved one has to meet Oklahoma's Medicaid rules first, and this is where a lot of families get anxious. For long-term-care coverage, a single applicant is limited to $2,000 in countable assets (the home, one vehicle, and household goods do not count) and $2,982 a month in countable income, which equals 300 percent of the 2026 federal benefit rate.3

Oklahoma is an income-cap state with no medically needy spend-down program, so being over the income limit does not automatically shut you out. An applicant whose monthly income tops $2,982 sets up a Qualified Income Trust (a Medicaid Income Pension Trust, often called a Miller Trust) and deposits the excess into it each month to qualify. That is the route for the over-income family that is not veteran-connected: a Miller Trust for the paid pathway, plus free NFCSP respite regardless of income while you sort it out.3

CDPASS also requires a nursing-facility level of care, the clinical gate confirming that without help at home your loved one would need a nursing facility. Oklahoma Human Services assesses that need as part of the ADvantage application.6

What Will the VA Pay an Oklahoma Caregiver?

If your loved one served, do not skip this section. Veterans' benefits often pay as well as or better than Medicaid, and they are the one place a spouse is paid outright rather than by exception.

VA Program of Comprehensive Assistance for Family Caregivers (PCAFC)

The PCAFC pays a monthly stipend to the Primary Family Caregiver of an eligible veteran. It is calculated from the federal General Schedule GS-4, Step 1 annual rate for the veteran's locality, divided by 12, then multiplied by a level factor (Level 1 is 0.625; Level 2 is 1.00 when the veteran cannot self-sustain in the community). The stipend is federal tax-free and can pay a spouse. To qualify, the veteran needs a service-connected disability rating of 70 percent or higher, a need for in-person personal care for at least six continuous months, and enrollment in VA health care.5

For a spouse who has been providing unpaid care for months while the household lives on one income, PCAFC is often the benefit that finally recognizes that work with a paycheck.

VA Aid and Attendance (A&A) Pension

Wartime veterans and surviving spouses who meet the functional criteria and fall under the net-worth limit ($163,699 in 2026) may receive the Aid and Attendance (A&A) pension. A single veteran with A&A receives up to $2,424 a month ($29,093 a year); a veteran with one dependent up to $2,874 a month. The pension goes to the veteran, who often pays a family caregiver from it.8

The Oklahoma Department of Veterans Affairs (ODVA) and county Veterans Service Officers help file these claims, so there is no reason to pay a for-profit consultant.7

Will I Owe Taxes on Caregiver Pay in Oklahoma?

Getting paid to care for someone you love raises a practical worry: will a tax bill eat into it? Here is the short version, though a tax preparer familiar with caregiver pay is worth a call before you file.

IRS Notice 2014-7 (Federal Exclusion)

If you live in the same home as the person you care for and are paid through a Medicaid home and community-based services waiver, your wages may be excluded from federal gross income under an IRS rule, Notice 2014-7. The exclusion applies when the caregiver and the care recipient share the same home as the caregiver's primary residence, and it does not apply when the caregiver keeps a separate home. This fits many Oklahoma CDPASS arrangements, so it is worth raising with your preparer.9

Oklahoma State Income Tax

Oklahoma's income tax starts from your federal adjusted gross income, so wages excluded federally under Notice 2014-7 generally flow through and are not taxed by the state either. Confirm the current treatment with the Oklahoma Tax Commission or a tax preparer.

The PCAFC monthly stipend is federal tax-free and is not reported on a W-2.5
