VA Senior Care Benefits in Alabama: Long-Term Care Options

The VA offers a menu of long-term and home- and community-based care programs for eligible veterans.1 Each has its own eligibility gate, so ask the VA social work department which ones the veteran qualifies for rather than assuming.

Home Based Primary Care (HBPC)

A VA doctor leads a health care team that comes to the veteran at home, providing services such as nursing care, physical therapy, and help with daily tasks.1 HBPC is aimed at veterans with complex medical needs who have difficulty getting to a clinic regularly.

Families often confuse this with home health care under Medicare; they are separate programs with separate rules, and your VA social worker can explain how the two would fit together in your case. For veterans who qualify, HBPC is one of the strongest programs the VA offers.

Adult Day Health Care

Veterans attend a daytime program offering social activities, companionship, recreation, care, and support.1 It also gives family caregivers reliable daytime respite. Ask your VA social worker which programs are available near you.

Community Living Centers (VA Nursing Homes)

Community Living Centers are VA nursing centers designed to feel like home, providing nursing-home-level care.1 Ask your VA medical center's social work department which CLC serves your area and what it can offer.

Community Nursing Home Program

The VA contracts with community (non-VA) nursing homes so veterans can get nursing home care near home.1 Whether the VA pays, and how much, depends on the veteran's eligibility, so ask the VA social work department before you assume a bed is covered.

Respite Care

Respite pays for someone to come into the veteran's home, or for the veteran to attend a program outside the home, while the family caregiver takes a break.1 All enrolled veterans are eligible for VA respite care if they meet the clinical criteria for the service and it is available; Nursing Home Respite Care is capped at 30 days per calendar year.6 Separately, under the VA Program of Comprehensive Assistance for Family Caregivers (PCAFC), eligible Primary Family Caregivers receive at least 30 days of respite care per year.6 Contact the VA Caregiver Support Line at 1-855-260-3274.6

Alabama State Veterans Homes

Alabama operates five State Veterans Homes under the Alabama Department of Veterans Affairs (ADVA).2 They serve aging or chronically disabled veterans, and ADVA describes them as a partnership between the State of Alabama, the federal government, and private industry: ADVA announced in 2022 that the four homes then operating had been run since 2004, under ADVA oversight, by HMR of Alabama, an Alabama-based provider of long-term care services for veterans, and its current home pages still direct employment inquiries to HMR.2 Services listed for the homes include 24-hour nursing coverage, a physician on call 24 hours, and physical, occupational, and speech therapy on site.2

Admission requires an honorable discharge with at least 90 continuous days of active-duty service (24 continuous months, or the full period called, for those who enlisted after September 7, 1980, or were commissioned after October 16, 1981), Alabama residency during the immediate past 12 months, and meeting the federal VA's criteria for skilled nursing care or domiciliary/assisted living.2 Note that this state-home service test is not the same as the federal Aid and Attendance wartime-service test below, and a veteran can meet one without meeting the other. VA Aid and Attendance

The Aid and Attendance pension is a monthly cash benefit for veterans (or surviving spouses) who need help with daily activities.3

2026 Rates

The VA publishes these as annual amounts; a monthly payment is the yearly award divided by 12.3 They are a ceiling rather than a payment: the Maximum Annual Pension Rate is the most the pension can pay, and the VA bases the actual award on the difference between your income for VA purposes and that limit.3

Who Qualifies

To be eligible, the veteran must not have received a dishonorable discharge, and generally must meet all four of these:7

• Wartime service. The VA lists three ways to meet it, and any one of them counts. A veteran who started active duty before September 8, 1980 needs at least 90 days of active duty with at least one day during a wartime period. A veteran who started active duty as an enlisted person after September 7, 1980 needs at least 24 months, or the full period for which they were called or ordered to active duty, a rule the VA itself qualifies "(with some exceptions)," again with at least one day during wartime. And a veteran who was an officer and started active duty after October 16, 1981 meets the test if they had not previously served on active duty for at least 24 months. A post-1980 veteran with fewer than 24 months is not automatically ruled out.7
• Meet at least one of the VA's four age-or-disability tests: be 65 or older; have a permanent and total disability; be a patient in a nursing home for long-term care because of a disability; or be getting Social Security Disability Insurance or Supplemental Security Income. Any single branch is enough, and the nursing-home and SSDI/SSI branches stand on their own, so a wartime veteran under 65 on SSI qualifies without any adjudicated rating.7
• Meet the VA's need-for-aid-and-attendance test. Any one of these does it: needing another person's help on a regular basis with activities such as dressing and undressing, keeping clean and presentable, feeding yourself, or attending to the wants of nature, or needing care or assistance regularly to stay safe from the hazards of daily life; having to stay in bed, or spend a large portion of the day in bed, because of illness; being a patient in a nursing home because of a loss of mental or physical abilities related to a disability; or eyesight of 5/200 or less in both eyes, or a visual field contracted to 5 degrees or less.7
• Have a net worth below $163,699, which counts the claimant's and dependents' assets and income for VA purposes; assets exclude the primary residence, the car, and basic home items like appliances3

The VA enforces a 3-year look-back period on assets transferred for less than fair market value, and a transfer that would have pushed net worth over the limit can trigger a penalty period of up to five years.7

How to Apply

The steps below are the pension route, the one the VA describes when it says "You may be eligible for this benefit if you get a VA pension."8

Apply using VA Form 21-2680 (Examination for Housebound Status or Permanent Need for Regular Aid and Attendance), with the examination section filled out by a medical examiner. That same form also covers Aid and Attendance "that will be added to your monthly compensation or pension benefits," so a veteran receiving VA disability compensation rather than a pension uses it too.8 A wartime veteran on the pension route who is not already receiving a VA pension also files Form 21P-527EZ (Application for Veterans Pension), which is the means-tested wartime pension application; it is not the form for a compensation-based claim.8 If you are still gathering information, you can file VA Form 21-0966 (Intent to File) first, because, in the VA's words, "Submitting an intent to file can secure the earliest possible effective date for any retroactive payments you may be eligible to receive."8

Asked how long a decision takes, the VA's answer is "It depends." It processes claims in the order it receives them, unless a claim requires priority processing.8

Veteran-Directed Care

Veteran-Directed Care (VDC) is a VA Geriatrics and Extended Care program, separate from the Aid and Attendance pension, that gives enrolled veterans a flexible budget, managed by the veteran or their representative, to hire and supervise their own workers so they can keep living at home or in their community.9 Veterans in the program may hire family, friends, and neighbors, including a spouse.9 Enrolled veterans are eligible if they are eligible for community care, meet the clinical criteria for the service, and it is available where they live.9

The veteran works with a person-centered options counselor at an Aging and Disability Network Agency, such as an Area Agency on Aging, and a financial management services provider helps handle the employer responsibilities that come with being the one doing the hiring.9 This is a good option for veterans who want control over their care and prefer family members as caregivers.

Community Care Through the MISSION Act

The VA MISSION Act of 2018 (Public Law 115-182) has, since 2019, let enrolled veterans get care from community (non-VA) providers at VA's expense.4 Two things always apply: the veteran has to be enrolled in or eligible for VA health care, and has to get approval from their VA care team before seeing a community provider, except in cases like urgent or emergency care.4 Beyond that, you may be eligible for community care if:

• The VA can't offer an appointment within 20 days (primary care/mental health) or 28 days (specialty care)
• The drive to a VA facility exceeds 30 minutes (primary care) or 60 minutes (specialty care)
• The care you need isn't available at your VA facility
• Community care is in your best medical interest

On May 19, 2025, VA implemented language from the Senator Elizabeth Dole Act (Public Law 118-210, enacted January 2, 2025) that removes the extra step of review by a second VA doctor on the best-medical-interest pathway, giving eligible veterans faster access to community care.4

For Alabama veterans in rural areas far from a VA medical center, the MISSION Act can be the difference between getting timely care and waiting months.

How Alabama VA Senior Care Benefits Work with Medicare and Medicaid

VA benefits don't replace Medicare or Medicaid: a veteran isn't required to enroll in Medicare to keep VA health care, and a veteran or surviving spouse can often receive VA pension and Alabama Medicaid at the same time.10,11 But they don't coordinate or pay each other: you pick which coverage to use each time you get care, and VA health care isn't creditable coverage for Medicare Part B, so delaying Part B while you rely on the VA alone leaves a lifetime penalty that grows every year you wait.10

• VA + Medicare: Many veterans carry both, and the two cover different care: Medicare is the payer for civilian providers, while VA care is delivered through VA and its contracted community providers. Ask the VA enrollment office how your specific care would be billed before assuming either one covers a given visit.
• VA + Medicaid: Aid and Attendance is paid as an increased monthly VA pension, and when a senior applies for Alabama Medicaid long-term care the state applies SSI income rules at the eligibility step, under which the Aid and Attendance allowance is not counted as income at all, so only the basic pension counts toward the income limit. Alabama is an income-cap state, with a 2026 cap of $2,982/month (300% of the Federal Benefit Rate), so applicants over the cap may need a Qualified Income Trust, often called a Miller Trust. That exclusion is an eligibility rule, not a permanent one: once someone is eligible and living in a nursing facility or other medical institution, income that was disregarded in determining eligibility must be considered in the post-eligibility share-of-cost calculation (42 CFR 435.725), so the Aid and Attendance amount does count at that stage. It does not all reach the facility, though. Before the state reduces its payment to the nursing home, 42 CFR 435.725(c) requires the agency to deduct five amounts from the resident's total income, in this order: (1) a personal needs allowance for clothing and other personal needs while in the institution; (2) for a resident with only a spouse at home, an amount for that spouse's maintenance needs; (3) for a resident with a family at home, an amount for the family's maintenance needs; (4) incurred expenses for medical or remedial care that no third party will pay, including Medicare and other health-insurance premiums, deductibles and coinsurance; and (5) the full amount of any SSI and state supplementary payments the resident continues to receive. All five are deductions the agency is required to make, not allowances a family has to apply for, and only what remains after them goes to the facility. No single national figure sets those amounts: Alabama sets the personal needs allowance above a federal minimum, and the spouse and family amounts come from a reasonable assessment of need measured against the state's own standards, so ask the Alabama Medicaid Agency for the figures in your case. That section governs care in a nursing facility or other medical institution; a person receiving home and community-based waiver services has a separate post-eligibility calculation under a different federal rule, so a waiver family should have the agency compute their share of cost rather than assume the nursing-facility arithmetic.12
• State Veterans Homes: ask ADVA which payment sources a given home accepts and how they combine, rather than assuming a particular mix.
