The social model is built for someone who needs supervision, structure, and company, but not much hands-on medical care. The medical model, usually called adult day health care, adds personal care, nursing oversight, medication management, and often physical or occupational therapy.2

When Medicaid, the VA, or a long-term care policy pays for "adult day," they are almost always paying for adult day health care, the medical model, for someone whose needs are significant enough to qualify. What Adult Day Care Costs in 2026

According to the CareScout Cost of Care Survey (formerly the Genworth survey, the standard reference for care costs), the national median for adult day health care in 2026 is about $95 a day. At five days a week, that's roughly $24,700 a year. The adult day care median actually slipped about 5% from the prior year, one of the few care settings that got cheaper rather than more expensive.1

Two things to hold onto before you budget from that number. First, it's a median, so your local rate can sit well above or below it depending on your region and the program. Second, the daily rate doesn't always include everything. Transportation to and from the center, meals, and specialized care (bathing help, extra therapy, a late pickup) can be billed on top, and some programs charge by the hour rather than a flat day. Ask any program you're considering for its full fee schedule, in writing, so the "day rate" you compare is really the whole cost.

Paying Out of Pocket

For a social-model program, paying yourself is the common path, and it's worth knowing it's rarely the sticker price. Many adult day centers are run by nonprofits or funded in part through the National Family Caregiver Support Program, which funds caregiver respite and related services through state and local agencies.3 Centers like these are often subsidized, so a listed rate of $95 a day may be well above what a qualifying family actually pays.1

Your local Area Agency on Aging is the fastest way to find those subsidized programs and any respite dollars that can offset the cost. You can find yours through the federal Eldercare Locator. It costs nothing to ask, and the family that assumes it can't afford day care often finds it can.

Does Medicaid Pay for Adult Day Care?

Yes, and for many families Medicaid is the answer, but it pays through a specific door with a real gate on it.

The door is a home and community-based services (HCBS) waiver. Under a Medicaid HCBS 1915(c) waiver, a state can cover adult day health services (along with things like personal care, homemaker help, and respite) as an alternative to putting someone in a nursing home. The whole point of the waiver is to keep people in the community, and adult day care fits that goal squarely.4

Now the gate, because you deserve to know it before you count on it. Waiver coverage is gated two ways. Your parent has to be assessed as needing an institutional (nursing-home) level of care, meaning their needs are serious enough that they could otherwise be admitted to a facility. And because each state caps how many people its waiver can serve, qualified applicants are frequently placed on a waiting list to enroll.5

That waitlist is the hard part, and it's not a reason to give up, it's a reason to apply early. Contact your state Medicaid agency or Area Agency on Aging the moment adult day care looks likely, get the level-of-care assessment scheduled, and get your parent's name on the list. A spot that opens in eight months does you no good if you apply in month seven.

Does Medicare Cover Adult Day Care?

This is the question that trips up the most families, so let's be plain: Original Medicare (Parts A and B) does not pay for adult day care. Medicare covers skilled, medically necessary care, not long-term custodial care, which is the non-skilled help with daily activities that adult day care provides. Medicare's own coverage guidance actually names adult day health care as an example of the long-term care it doesn't cover.6

If you've been told otherwise, it was probably about one of these narrower channels, each with its own limit:

• A Medicare Advantage plan. Some Medicare Advantage plans offer adult day services as a supplemental benefit beyond what Original Medicare covers.7 It's entirely plan-specific, so you have to check that plan's benefits for the coming year; most plans don't include it.
• PACE. The Program of All-Inclusive Care for the Elderly covers everything Medicare and Medicaid cover, adult day care included, plus whatever else its care team decides you need. The one condition is that PACE is all-or-nothing: you have to enroll in PACE for all your care, and you must be certified as needing a nursing-home level of care to join.8
• The CMS GUIDE Model. This newer Medicare dementia program gives participating providers a respite allowance of up to $2,500 a year that families can put toward adult day care.9 It's dementia-only, and only where your parent's provider takes part, so ask them directly.

What About the VA?

If your parent is a veteran, the VA is one of the better answers here. Adult Day Health Care is one of the long-term care programs the VA offers to eligible veterans, right alongside home-based primary care and respite care.10 What a veteran pays depends on their VA priority group and service-connected status, so the copay isn't the same for everyone.

Insurance and a Tax Break That Can Help

Two more sources are easy to overlook.

Long-term care insurance. If your parent has a long-term care policy, adult day care is often a covered benefit under its home and community-based care terms, sometimes at a more generous daily limit than home care. Policies vary a lot, so read the actual benefit language or call the insurer before assuming either way.

A federal tax credit. If adult day care lets you (or your spouse) stay employed, its cost may count toward the Child and Dependent Care Credit. There's a specific rule worth knowing: care provided outside your home, which adult day care is, counts as a work-related expense only if the disabled adult you're caring for regularly spends at least 8 hours a day in your home.12 A tax professional can tell you whether your situation fits.

How to Lower Your Adult Day Care Cost

The cost of adult day care has more give in it than the day rate suggests. A few moves that consistently help:

• Match the model to the need, and no more. If your parent needs supervision and company but not nursing care, a social program costs less than adult day health care and serves them just as well. Paying for the medical model when the social one fits is money spent for nothing.
• Apply for Medicaid or the VA before you think you need to. Both run on waitlists and assessments that take time. Early is free; late costs you months of private pay.5
• Ask every program for its full fee schedule in writing. Confirm what the day rate includes, whether transportation and meals cost extra, and whether there's a sliding scale, before you compare two centers on price.
