What Adult Day Care Is, and Why the Type of Program Matters

Adult day care, which many families search for as senior day care or elderly day care, gives an older adult somewhere to go during the day, with meals, activities, supervision, and companionship, while a family caregiver works or simply gets a few hours back. For many families it's part of what makes keeping a parent at home possible.

Before you look at prices, know that adult day programs are generally grouped into three models. The social model is built for someone who needs supervision and activities, but not extensive personal care or medical monitoring. The medical model, usually called adult day health care, adds more extensive personal care, medical monitoring, and rehabilitative services, and states generally hold it to stricter requirements, particularly for nurse staffing. A third, specialized model serves people with a specific diagnosis, most often dementia.

Why does that matter when you're paying? Because much of the help is named for a specific service. Under a Medicaid home and community-based services waiver, a state may cover adult day health services as an alternative to a nursing home, and only for someone assessed as needing an institutional level of care. What Adult Day Care Costs in 2026

According to CareScout's 2025 Cost of Care Survey, released in March 2026 by Genworth Financial, which owns CareScout, the national median for adult day health care is about $95 a day. At five days a week, that's roughly $24,700 a year. That median actually slipped 5% from the prior year. It's one company's survey of provider rates, with no second, independent national survey behind it.

Two things to hold onto before you budget from that number. First, it's a median of what providers charge, not what a family nets out paying, so your local rate can sit well above or below it depending on your parent's care needs, the program, and your local market. Second, ask what the daily rate actually includes: whether transportation, meals, or extra help like bathing are billed separately, and whether the program charges by the day or by the hour. Get any program's full fee schedule in writing, so the "day rate" you compare is really the whole cost.

Even at the median, adult day health care costs less than paid in-home help or assisted living. In the same CareScout survey, about $95 a day buys less than three hours of a non-medical in-home caregiver at the $35-an-hour national median, and a year at five days a week, about $24,700, is roughly a third of the $74,400 national median for a year of assisted living. Many families end up combining sources rather than finding one payer that covers it all. Here's how the main payers line up, so you can see where you're likely to land before reading the detail.

Paying Out of Pocket

If no program covers your parent's days, paying yourself is the default, but the listed rate isn't always the whole story. The National Family Caregiver Support Program funds respite care and other caregiver services through states and their area agencies on aging. Its respite help is for caregivers of someone who is frail, meaning they can't do at least two daily activities without substantial help, or, because of a cognitive or other mental impairment, need substantial supervision because their behavior poses a serious health or safety hazard. That respite can be provided in an adult day care setting. And the $95 median is a provider rate, not a family's net cost, so what a qualifying family actually pays can differ from the listed rate.

Does Medicaid Pay for Adult Day Care?

It can, but only through a specific door, and there's a real gate on it.

The door is a home and community-based services (HCBS) waiver. Under a Medicaid HCBS 1915(c) waiver, a state can cover adult day health services (along with things like personal care, homemaker help, and respite) as an alternative to putting someone in a nursing home. The whole point of the waiver is to keep people in the community, and adult day care fits that goal squarely.

Now the gate, because you deserve to know it before you count on it. Waiver coverage is gated two ways. Your parent has to be assessed as needing an institutional level of care, meaning that without waiver services they would need the level of care a nursing facility, a hospital, or an intermediate care facility for people with intellectual disabilities provides. And because each state sets a limit on how many people its waiver serves, qualified applicants can be placed on a waiting list to enroll.

That waitlist is the hard part, but it's a reason to apply early, not a reason to give up. Contact your state Medicaid agency or Area Agency on Aging the moment adult day care looks likely, ask how the level-of-care assessment works, and get your parent's name on the list. A wait only starts once they're on it.

Does Medicare Cover Adult Day Care?

This question trips up a lot of families, so let's be plain: Original Medicare (Parts A and B) does not pay for adult day care. Medicare doesn't pay for long-term care, which it also calls custodial care: mostly non-medical help with the basic personal tasks of everyday life. Medicare's own long-term care page lists adult day health care among the services long-term care includes, and says you pay all costs for most long-term care.

If you've been told otherwise, it was probably about one of these narrower channels, each with its own limit:

  • A Medicare Advantage plan. Most Medicare Advantage plans offer extra benefits Original Medicare doesn't, such as routine dental, vision, hearing, and fitness. Those extras are plan-specific, so check that plan's benefits for the coming year to see whether adult day services are among them.
  • PACE. The Program of All-Inclusive Care for the Elderly covers all Medicare-covered services and all Medicaid-covered services in your state's plan, plus whatever else its care team decides is necessary, and adult day care is part of it. To join, your parent must be 55 or older, live in a PACE organization's service area, be found by the state to need a nursing-home level of care, meet any other conditions in the local PACE program's agreement, and be able to live safely in the community when they enroll. PACE is only available in some states.
  • The CMS GUIDE Model. In this Medicare pilot program for dementia, CMS reimburses participating providers up to $2,500 a year per eligible patient for respite services that give qualifying caregivers a break, and adult day center programs are one of the respite options., It isn't cash paid to your family, and it's only available where your parent's provider takes part, so ask them directly.

What About the VA?

If your parent is a veteran, the VA is one of the better answers here. Adult Day Health Care is one of the long-term care programs the VA offers to eligible veterans, right alongside home-based primary care and respite care. Whether a veteran owes a copay depends on their situation. For care that isn't for a service-connected disability, a veteran without a compensable service-connected disability generally has to agree to a copay for days of care beyond the first 21 in a year, with exceptions that include veterans whose income is below the VA pension income limit.

On top of that, a veteran who gets a VA pension and meets at least one VA Aid and Attendance requirement, such as needing another person's help with daily activities like bathing, feeding, and dressing, may be eligible for an increased monthly payment added to that pension, and that money can go toward adult day care. Insurance and a Tax Break That Can Help

Two more sources are easy to overlook.

Long-term care insurance. If your parent has a long-term care policy, adult day care may be a covered benefit. These policies are built to pay for personal and custodial care in settings that include the home and community organizations, and most pay once someone needs help with two or more daily activities or has a cognitive impairment. Policies vary a lot, so read the actual benefit language or call the insurer before assuming either way.

A federal tax credit. If adult day care is what lets you keep working, its cost may count toward the Child and Dependent Care Credit. Two rules apply because adult day care happens outside your home. The person you're caring for, a spouse or dependent who can't care for themselves, has to regularly spend at least 8 hours a day in your home. And if the program counts as a dependent care center, meaning it cares for more than six people who don't live there and is paid a fee, payment or grant, it has to comply with all applicable state and local laws. The cost of adult day care has more give in it than the day rate suggests. A few moves that consistently help:

  • Start with your Area Agency on Aging. Through the Eldercare Locator you can reach your local agency, which can tell you what caregiver support, including respite help, is available near you.
  • Match the program to the need. The social model is built for someone who needs supervision and activities but not extensive personal care or medical monitoring. If that describes your parent, compare social programs' rates with adult day health care's before you choose.
  • Apply for Medicaid before you think you need to. The waiver runs on a level-of-care assessment and can come with a waiting list. Early is free; late can cost you months of private pay.
  • Ask every program for its full fee schedule in writing. Confirm what the day rate includes, whether transportation and meals cost extra, and whether there's a sliding scale, before you compare two centers on price.
