Paying for Senior Care in Idaho From Your Own Money
Start with your parent's own money: Social Security, a pension, savings and sometimes the house. Adding those up tells you how many months private pay can cover, and so how early to start a Medicaid application. Idaho's tax rules decide how much of that income your parent actually keeps.
What Idaho Taxes, and What It Doesn't
For the family budget, the good news comes first: Idaho does not tax Social Security benefits, and Railroad Retirement Board benefits are exempt from Idaho income tax as well, so a parent living mainly on Social Security owes Idaho no income tax on that check.tax.idaho.gov. The harder news is for a parent drawing on a private pension or retirement account. Private pensions, IRA withdrawals and 401(k) distributions get no Idaho retirement-benefits deduction, and the Idaho State Tax Commission publishes Idaho's flat individual income tax rate as 5.3 percent of Idaho taxable income for 2025.tax.idaho.gov. A few pensions are the exception. Idaho's Retirement Benefits Deduction covers only federal CSRS (not FERS) and Foreign Service pensions, retired Idaho city police officers' benefits, the Idaho Firefighters' Retirement Fund, generally for a taxpayer at least 65, or at least 62 and disabled. Federal Tax Breaks When You Pay for Care
Federal tax rules can return some of what the family spends. A taxpayer who itemizes on Schedule A can deduct unreimbursed medical expenses only to the extent they exceed 7.5% of adjusted gross income, so a family that takes the standard deduction gets no medical deduction. Claiming a parent as a dependent for 2026 requires, among other federal tests, that the parent's gross income be less than $5,300 and that you provide over half their support or qualify under a multiple support agreement. Even when a parent's income is too high to claim them, you can still include the medical costs you paid for them in your own deductible medical expenses, provided they would have been your dependent except for the gross-income, joint-return, or claimed-as-someone-else's-dependent tests.Internal Revenue Service. One HECM rule decides whether a reverse mortgage fits your family. If a HECM reverse-mortgage borrower is away for more than 12 consecutive months in a healthcare facility such as a nursing home or assisted living facility, and no co-borrower lives in the home, anyone living there must move out unless they can repay the HECM or qualify as an Eligible Non-Borrowing Spouse. The Five-Year Look-Back
Under federal Medicaid law, a 60-month look-back applies to uncompensated asset transfers, and the 60 months run backward from the Medicaid application, not from the gift. What Happens to the House
Whether the house truly helps pay for care depends on what the state can claim later. Under federal Medicaid law, a state must seek recovery from the estate of a person who was 55 or older when they received Medicaid, at least for nursing facility services, home and community-based services and related hospital and prescription drug services. Federal law sets the timing: Medicaid estate recovery may be made only after the death of a surviving spouse, and only when there is no surviving child who is under 21 or blind or permanently and totally disabled. Long-Term Care Insurance
If your parent bought a long-term care insurance policy years ago, find it now and read the benefit triggers, the daily maximum and the elimination period. Per-diem benefits from a tax-qualified long-term care insurance policy are excluded from federal income only up to the greater of an indexed amount, $430 a day for 2026, or the costs actually incurred for care. Tax-qualified long-term care insurance premiums count as a federal medical expense only up to age-banded limits, which for 2026 are $4,960 for someone more than 60 but not more than 70 and $6,200 for someone over 70, and through the itemized deduction they reduce tax only for medical expenses above 7.5 percent of adjusted gross income.Internal Revenue Service.
