Paying for Senior Care in Nevada From Your Own Money
Social Security, a pension, savings and the house are where a Nevada care budget begins, and Nevada's tax code takes none of that income at the state level. The Nevada Department of Taxation states that the State of Nevada does not impose a state income tax on individuals, and the Nevada Constitution provides that no income tax shall be levied upon the wages or personal income of natural persons. Nevada retirees still owe federal income tax, and Nevada does levy other taxes, such as sales tax and property tax.tax.nv.gov. (n.d.). A statutory partial abatement under NRS 361.4723 caps the annual increase in the property tax bill on an owner-occupied primary residence in Nevada at 3 percent over the prior year's bill. The Nevada 3 percent cap is not automatic: it must be claimed, either on a form from the county assessor or on the declaration-of-value form. An owner does not lose the Nevada 3 percent cap by placing the home's title in a trust for estate-planning purposes while still living there.Nevada Legislature. (n.d.). Borrowing Against the House
Every route to borrowing against the house is secured by the home and reduces the equity the owner or the heirs would otherwise keep, and a home equity loan or line of credit usually carries monthly payments.Consumer Financial Protection Bureau. If a HECM borrower is away for more than 12 consecutive months in a healthcare facility such as a nursing home or assisted living facility and no co-borrower lives in the home, anyone else living there must move out unless they can repay the loan or qualify as an Eligible Non-Borrowing Spouse. For that reason, a HECM fits care delivered at home, or a couple where one spouse stays in the house, far better than one person's permanent move into a facility.Consumer Financial Protection Bureau. What Medicare Pays For, and What It Won't
Medicare, in Nevada as everywhere, does not cover custodial care, the non-medical help with bathing, dressing and using the bathroom, when that is the only care a person needs, which is why Medicare does not pay for a long-term stay in a nursing home or an assisted living facility.Centers for Medicare & Medicaid Services. (n.d.). Long Term Care Coverage. medicare.gov. Retrieved Jul 30, 2026, from https://www.medicare.gov/coverage/long-term-care
Medicare Part A pays for skilled nursing facility care on a short-term, post-acute basis, up to 100 days per benefit period, and it generally requires a qualifying inpatient hospital stay of at least three consecutive days first. Time under observation or in the emergency room before admission does not count toward Medicare's three days, even overnight. A patient whose doctor participates in an Accountable Care Organization approved for a Skilled Nursing Facility 3-Day Rule Waiver may not need the three-day stay, and a Medicare Advantage plan may also waive it. A Medicare patient whose hospital changes their status from inpatient to outpatient observation can ask for a fast appeal while still in the hospital.Centers for Medicare & Medicaid Services. (n.d.). SNF Care Coverage. medicare.gov. Retrieved Jun 23, 2026, from https://www.medicare.gov/coverage/skilled-nursing-facility-care
In 2026, a Medicare Part A skilled nursing facility stay costs $0 a day for days 1 through 20 after the $1,736 deductible, which is not owed again if you already paid it for hospital care in the same benefit period; $217 a day for days 21 through 100; and after day 100, Medicare pays nothing.Centers for Medicare & Medicaid Services. How Nevada Medicaid Pays for Long-Term Care
Nevada Medicaid is the program that pays for a long nursing-home stay once a resident qualifies. Nevada Medicaid covers nursing-facility care, which is a mandatory Medicaid benefit, for people who need more care than room and board alone.Centers for Medicare & Medicaid Services. (n.d.). Estate Recovery. medicaid.gov. Retrieved Sep 6, 2026, from https://www.medicaid.gov/medicaid/eligibility-policy/estate-recovery
The money test a Nevada Medicaid applicant is measured against uses the federal 2026 SSI and Spousal Impoverishment Standards, effective January 1, 2026: a Supplemental Security Income (SSI) Federal Benefit Rate for an individual of $994 a month, an income cap of $2,982 a month (300% of that $994 rate), and an SSI resource standard of $2,000 for a single applicant.Centers for Medicare & Medicaid Services. (n.d.). Estate Recovery. medicaid.gov. Read the Nevada income rules in full before accepting a verbal answer that your parent's income is over the limit.
Once a Nevada nursing-home resident is on Medicaid, the federal post-eligibility rules decide how much the resident is able to contribute to the cost of his or her own care, and those federal rules protect a personal needs allowance for the resident.Centers for Medicare & Medicaid Services. (n.d.). Estate Recovery. medicaid.gov. Retrieved Sep 6, 2026, from https://www.medicaid.gov/medicaid/eligibility-policy/estate-recovery Nevada's own figure is in our guide to the Nevada Medicaid personal needs allowance.
Home equity has its own federal ceiling. Under federal Medicaid law, an applicant whose home equity exceeds the 2026 minimum of $752,000 is not eligible for long-term care assistance, and a state may substitute a higher figure up to $1,130,000. The federal Medicaid equity limit does not apply at all while a spouse, a child under 21, or a child who is blind or permanently and totally disabled lawfully lives in the home.Office of the Law Revision Counsel, U.S. House of Representatives. (2026). 42 U.S.C. 1396p(f) - Disqualification for long-term care assistance for individuals with substantial home equity, including the (f)(2) exception, the (f)(3) reverse-mortgage route and the (f)(4) hardship waiver (uscode.house.gov prelim view, rolling edition; text contains those laws in effect on September 20, 2026). uscode.house.gov. Our guide on how to apply for Nevada Medicaid walks through the forms and offices, and Nevada Medicaid and nursing home care covers the nursing-home side in depth. If Nevada denies or reduces your parent's coverage, Nevada Medicaid appeals and fair hearings explains how to challenge the decision.
The Five-Year Look-Back
If your parent gave money or property away, or sold it for less than it was worth, in the five years before applying, Nevada Medicaid can refuse to pay for nursing-home or waiver care for a stretch of months. Under federal Medicaid law, which binds Nevada, that look-back reaches 60 months before the first date the person is both institutionalized and has applied for Medicaid, for any transfer made on or after February 8, 2006. A transfer for less than fair market value by the person or the person's spouse triggers a penalty period covering nursing facility services, an equivalent level of institutional care, and waiver home and community-based services.Centers for Medicare & Medicaid Services. (n.d.). Estate Recovery. medicaid.gov. Retrieved Sep 6, 2026, from https://www.medicaid.gov/medicaid/eligibility-policy/estate-recovery The penalty period is calculated by dividing the total uncompensated value by the state's average monthly private-pay cost of nursing-facility care, and it is not a denial of all Medicaid coverage.Office of the Law Revision Counsel, U.S. House of Representatives. (2026). 42 USC 1396p - Liens, adjustments and recoveries, and transfers of assets (OLRC, U.S. Code preliminary release; text contains those laws in effect on August 1, 2026). uscode.house.gov. Retrieved Sep 21, 2026, from https://uscode.house.gov/view.xhtml?req=granuleid:USC-prelim-title42-section1396p&num=0&edition=prelim
Not every transfer is penalized. Under federal Medicaid law, a home can pass without penalty to a spouse; to a child under 21 or a child who is blind or permanently and totally disabled; to a sibling who has an equity interest in the home and lived there at least one year immediately before the parent's institutionalization; or to a son or daughter who lived there at least two years immediately before the parent's institutionalization and, as determined by the state, provided care that let the parent stay home rather than in a facility.Office of the Law Revision Counsel, U.S. House of Representatives. (2026). 42 USC 1396p - Liens, adjustments and recoveries, and transfers of assets (OLRC, U.S. Code preliminary release; text contains those laws in effect on August 1, 2026). uscode.house.gov. Retrieved Sep 21, 2026, from https://uscode.house.gov/view.xhtml?req=granuleid:USC-prelim-title42-section1396p&num=0&edition=prelim Because each exception carries conditions, a Nevada family should get a Nevada elder-law attorney's written opinion before retitling the house or moving savings.
What Happens to the House
Whether the house is really a payment source depends on what Nevada can claim afterward. Nevada's Medicaid Estate Recovery Program applies to recipients who are 55 or older or who are inpatients of a medical facility, and it can recover home and community-based services, nursing facility services, hospital, physician and prescription drug services, managed care premiums and other Medicaid payments; Nevada may also place a lien against the property of a deceased recipient.Centers for Medicare & Medicaid Services. (n.d.). Estate Recovery. medicaid.gov. Retrieved Sep 6, 2026, from https://www.medicaid.gov/medicaid/eligibility-policy/estate-recovery Federal law sets the timing: Medicaid estate recovery may be made only after the death of a surviving spouse, and only when there is no surviving child who is under 21 or blind or permanently and totally disabled. Every state Medicaid agency must also have procedures to waive recovery where it would work an undue hardship.Office of the Law Revision Counsel, U.S. House of Representatives. (n.d.). 42 U.S.C. §1396p(b)(1) chapeau — the prohibition on recovery of correctly paid medical assistance and the three mandatory exceptions (Office of the Law Revision Counsel, U.S. Code, prelim edition). uscode.house.gov. Assisted Living and the Frail Elderly Waiver
Nevada licenses assisted living inside its license for a residential facility for groups, and before a person moves in, a Nevada assisted living facility must make a full written disclosure of the personalized care services available and the amount it will charge for them throughout the stay.Nevada Legislature. (2026). Nevada Revised Statutes chapter 449 (Rev. 9/9/2026) — NRS 449.017 ("residential facility for groups" defined) and NRS 449.0302(7) (conditions on claiming to provide assisted living services). leg.state.nv.us. Retrieved Sep 25, 2026, from https://www.leg.state.nv.us/NRS/NRS-449.html Keep the facility's written disclosure, because the Nevada facility's own charges are the figure the rest of this plan has to cover.
Nevada Medicaid's Waiver for the Frail Elderly is the Nevada Medicaid program that can pay for care in assisted living, and its conditions are strict. Under Nevada's Medicaid Services Manual, a Frail Elderly Waiver applicant must be 65 or older, must meet the level of care for admission into a nursing facility, and would require imminent placement in a nursing facility, within 30 days or less, if home and community-based services or other supports were not available. Two agencies share the decision: the Nevada Division of Welfare and Supportive Services decides Medicaid financial eligibility, and the Nevada Aging and Disability Services Division determines Frail Elderly Waiver eligibility together with the Division of Welfare and Supportive Services.Nevada Legislature. (2026). Nevada Revised Statutes chapter 449 (Rev. 9/9/2026) — NRS 449.017 ("residential facility for groups" defined) and NRS 449.0302(7) (conditions on claiming to provide assisted living services). leg.state.nv.us. Long-Term Care Insurance
If your family member bought a long-term care insurance policy, find it and read the benefit triggers, the daily maximum and the elimination period. Per-diem benefits from a tax-qualified long-term care insurance policy are excluded from federal income only up to the greater of an indexed amount, $430 a day for 2026, or the costs actually incurred for care. Tax-qualified long-term care insurance premiums count as a federal medical expense only up to age-banded limits, and through the itemized medical-expense deduction they reduce tax only for the part of medical expenses above 7.5 percent of adjusted gross income.Internal Revenue Service. Frequently Asked Questions
Does Nevada tax Social Security or pensions?
No. Because Nevada imposes no state income tax on individuals, a Nevada retiree's care budget loses nothing to state income tax on Social Security or a pension; the tax to plan for is federal income tax, which Nevada retirees still owe.tax.nv.gov. Does Medicare pay for a nursing home in Nevada?

Will Nevada Medicaid pay for assisted living?
Partly. Nevada Medicaid's Frail Elderly Waiver pays for Augmented Personal Care in a Nevada assisted living facility for a person 65 or older who meets nursing-facility level of care, so a family budget for Nevada assisted living under the waiver still has to cover rent and meals. Because the Nevada Frail Elderly Waiver has a limited number of slots each year and a wait list, apply before savings run out.Nevada Legislature. (2026).
